- All participants and/or contact persons MUST CALL Bright Days Recreational Services at: 413-529-0250 to register for a vacation trip. New participants will be screened prior to registration.
- All requested reservations will be held for 10 business days. Your reservation will be confirmed upon receipt of completed paperwork and a 50% deposit for vacation trips. If additional time is needed please call our office at 413-529-0250.

- All paperwork must be filled out completely. PLEASE do not leave blank spaces and please print legibly.
- Obtain all required signatures for the consent agreement
- Attach a copy of the participant’s health insurance card and current medication list.
- Return ALL completed paperwork and 50% deposit to us within 10 business days.
- A receipt for your deposit and any balance due will be mailed to the indicated contact person. There will be a $10.00 service charge for incomplete or late paperwork.
