• All participants and/or contact persons MUST CALL Bright Days Recreational Services at: 413-529-0250 to register for a vacation trip. New participants will be screened prior to registration.
  • All requested reservations will be held for 10 business days. Your reservation will be confirmed upon receipt of completed paperwork and a 50% deposit for vacation trips. If additional time is needed please call our office at 413-529-0250.
  • You must download needed paperwork from our website (see ‘printable forms’ tab at top of page)

  • All paperwork must be filled out completely. PLEASE do not leave blank spaces and please print legibly.
  • Obtain all required signatures for the consent agreement
  • Attach a copy of the participant’s health insurance card and current medication list.
  • Return ALL completed paperwork and 50% deposit to us within 10 business days.
  • A receipt for your deposit and any balance due will be mailed to the indicated contact person. There will be a $10.00 service charge for incomplete or late paperwork.
